  • New Patients: Schedule below and complete all steps of your intake directions. After completing all of the steps of your intake directions, send a P2P portal message to Michelle Kosvitch and ask to be added to the wait list. Michelle will contact you if an earlier appointment becomes available.
  • Existing Patients: You will have the option to join the wait list while scheduling below.
• To CANCEL or RESCHEDULE, log in at the top, right corner of this page or by using the link in your appointment confirmation email.
• Use your legal name when scheduling instead of a nickname. Please use the same name as your Power2Patient account.
